Patient Manager Overview

Patient Manager is used by everyone in the health system to improve throughput and capacity, to drive multidisciplinary rounds, and to highlight barriers to care progression and discharge planning. Highly-customizable profiles give users the ability to pinpoint specific patient populations such as Long LOS, Discharge Lounge candidates, Complex Post-Acute Placements, Observation Management risks and many others.

Notes:

The Patient Manager Tile is used for multidisciplinary rounds, huddles, ad hoc problem-solving, and expediting critical activities. In the MDR profile, the care team can quickly review patient demographics, critical dates and flags, discharging disposition, pending or completed tasks, and notes.

In rounds, the care team can click the MD present button to indicate the presence of a provider. Many automated dates and tasks are pulled directly from the EMR. Additionally, the care team can update the patient's anticipated date of discharge, identify any new tasks or barriers, comments on tasks, complete tasks, and add notes.

Additionally, users can create customized profiles within the Tile to support many different use cases, such as patients who are missing ADD’s, discharging patients pending imaging, or discharging patients pending therapy, just to name a few. This is the Patient Manager Tile.
