#20 - Patient Manager Tile with Downgrade Module

Jeff Terry: Hello again. I'm Jeff Terry.

Let's take a look at, a closer look today at Patient Manager, which of course is accessed from Tile Viewer. I've landed where we left off last time, which I've got a profile you notice in the upper, right, which is a discharge planning and rounding type profile. In this case, I'm filtered to patients who have a discharge order for today or tomorrow. And their discharged disposition is to home with, or without home care. You can see 175 of our 2200 patients meet those filters.

By the way, all the data here is fake, but it is representative of what we see in practice. You could ignore that the dates are a little goofy because it's fake. And a reminder on the left, we have basic patient information, flags. To the right of that, we have key dates about the patient. To the right of that, the dispositions that we're following.

To the right of that, all of the tasks for that patient driven by orders that have been ordered and those that are completed If I hover, I can get more of information. All of this information is *real-time.

So if something changes in the order system or the pharmacy system or the lab system or the ADT system, it will update while we're looking at it. And with each of these, I can click and interact and I can add a task which will show up with a white border instead of a blue border. We can update the district positions that we're following, et cetera.

But today I want to drill into another capability of the Tile. And that's if I add the downgrade module. And to start with, I'll show you a profile, or that could be useful, let's say I need neuro ICU capacity and I need it right now. What patients may be ready for downgrade? And that's what this profile that I've moved to is for.

And so you see in this now middle of the Tile, I've got a column that's focused on downgrade readiness. The green bars mean that the algorithm, and that algorithm is from the medical staff, it’s not a GE algorithm, but the algorithm from the medical staff says these patients may be ready for downgrade to an acute or a medicine type unit.

If I scroll down further, you'll see some that the algorithm says, maybe ready for downgrade to an intermediate level of care. And off to the right, you can see the criteria that we're following that are causing that.

You see the rounded edges on those boxes, tell us that they're criteria and not tasks. And in this case, the patient requires hourly neurological checks and they're on one base of active medication. If I click, I can see the dosage, when would it be ordered, et cetera.

So that profile is useful for finding neuro ICU capacity. I want to show you another, which may be how the nurse manager of the neuro ICU uses the Tile. Instead of sorting by downgrade readiness, she just has it sorted by bed. That’s seven, bed eight, bed nine. And you can see, again, for each patient, what's their readiness status to the right, what tasks are open. There's a check if the task has been completed. And if I scroll down, you can see this mix of patients that are on the unit. Let's take a look, a closer look, at the patient in bed 11.

You can see that patient is not eligible for downgrade. You can see for every patient, we have the Glasgow coma **score and we have the Richmond agitation and sedation **score. The Tile can show up to two acuity **scores that are selected by the medical staff. Up to you entirely, which we use there.

And then to the right, on this patient, you can see there are three criteria that are keeping this patient in the neuro ICU. The first is that they have an endovascular device, and the second is that they have a neuromuscular blockade and you can see what medication that is. And the third is that they're on several, actually, vasoactive medications. And if I click and hover, I can see what they are, the dosages, the order, et cetera. As always, below that I can see the tasks that are open and those that have been completed.

So you can imagine how useful that is for the nurse manager of the neuro ICU to have all this information at once so that she can manage her patients for any number of topics, whether that's during rounds or looking for capacity or problem solving or all the other types of activity.
