#03 - Seniors Care Tile

In this Episode:

The Seniors Care Tile helps ensure geriatric inpatients outside the geriatric unit receive best practice care in accordance with hospital protocols.

Andy: Hi, this is Andy Day from the Command Center Team.

Today, we’re going to take a look at the Seniors Care Tile that provides visibility to geriatric patients who would benefit from being put on a specialized care plan. Designed by your clinicians with extensive therapies to enhance their restorative capabilities as quickly as possible. Where we are right now is on Tile Viewer, which is our landing page. The place where you land whenever you log into our system, and a variety of Tiles are available to you depending on your installation. So today we’re going to talk about seniors, so let’s click there and dive into the Tile.

Before I look at the individual patients that are alerting, let me just orient to the Tile a little bit and how we navigate. On the top left corner you see the name of the Tile, Seniors Care. Moving across to the right, you see the little ‘i’ or Info Button and that is where our Legend lives. So you can always see any of the symbols or flags that show on any Tile, you don’t need a cheat sheet or a reference guide. It’s always kept up to date and it’s right on the Tile itself and you can look at it whenever you want.

And then to the right of that, is the little Viewer Settings button, and this is where a user can adjust the Tile to the way they like to look at it and save their settings so it’s that way when they come back to it the next time. We’ll dive into this a little bit more in a second. And at the bottom left corner, all the way at the bottom, you see these two little blinking lights there, that’s the Freshness Indicator telling us when this Tile last got data from the system.

So it’s always fresh and current and we know it. Now again, we don’t enter any new data for this Tile, this is pulling data out of your existing systems and infrastructure and helping to put that data to work. So let’s now look at what actually puts a patient on the Tile and look at the rest of the body of the Tile itself. So each patient that’s showing here on the Tile is alerting because something in their care plan that was provided by your hospital is not staying on plan, so that causes patients to alert.

If you look at the section across the bottom that says, ‘Current Census’, this shows where the patients are in the hospital that are candidates for this accelerated care plan. And you see for example, there’s twenty in medicine, and you can click on that and see who the individual patients are and more information about how long they’ve been in the hospital. And so you can again, see at a glance where those different groups or clusters of these patients are.

Then across the top, are the actual patients who are alerting. So each one of these is a patient where they’ve deviated from that care plan. And so if you click on the patient, you can see information about the patient themselves, as well as who that patient’s most responsible nurse is right now and how to contact them. Now I should mention, of course, that this data is mock. It’s not real, obviously, but it is representative of what we see in the Tile in use and practice.

And then to the right of that patient information, you see individual alert icons. These are individual care plan elements, defined by your hospital, that are not staying on plan. So you see a mobility related alert, you see a GI related alert, which can often times cause other care plan activities to not be able to happen.

You see a medication related alert. You can see nutrition and other. And again, each of these individual alerts when they occur, will cause the patient to show on the Tile so we can see that we are falling off of plan. You’ll notice on the right hand side, a little pencil. This is where if you or your team wants to snooze or dismiss a patient that’s alerting on the Tile, you can snooze or dismiss an individual care plan activity that you decide is not appropriate or to snooze or dismiss the entire encounter off of this accelerated care plan Tile.

And so you can choose what you like. We’re asking you to put in a reason code. If you’re snoozing, we ask you to put in a duration, you can pick a time threshold. And then we ask you to explain why you’re doing it, because then we track that so that you can later look and see why patients are getting snoozed and dismissed. Often it’s because we’ve addressed this in some other way that’s not representative in the EMR.

And then we’d hit apply but we’ll cancel in this case. So this Tile is often used by the gerontology team or the quality people that are looking across the whole hospital to make sure that the patients that are on this accelerated care plan are receiving the care that’s intended in a timely fashion. It’s also often times used by a service manager of an individual area.

So let’s say I’m the dietary or nutrition manager, and I want to make sure that this population is receiving the care it needs. So I’ll go up into the gear button and apply a filter for nutrition related alerts. And by doing this, I can save this to my individual profile so next time I come it’s just this way, but you’ll see, these are the four patients amongst that whole population who are alerting for nutrition related reasons.

And so now I can focus on rounding on these patients and making sure that I address whatever care plan issue is occurring in a prompt fashion.

So this Tile, the Seniors Care Tile, is really intended to help hospitals have visibility to those patients that are most vulnerable and making sure they stay on their care plan.
